**Responsibilities**

As a Licensed Clinical Social Worker (LCSW), your role is specialized in providing clinical services, psychotherapy, and mental health interventions to individuals, families, and groups. The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients and emergency department patients. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving.

Full time social worker to provide direct patient care coordination/counseling (0.4 FTE) and support Residency Education (0.6 FTE) as Behavioral Health Faculty. This position provides an opportunity to serve a vital role in a vibrant program and academic department with positive culture.

Residency Behavioral Science Educator to provide advanced level of clinical care coordination services to patients and families presenting at the family medicine residency clinic with psychosocial needs across the continuum of care. See patients in practice independently as well as with the residents in co-coordination experiences. Adhere to all department and facility procedures regarding documentation in the medical records.

Elements of Residency Education include: development of lecture curriculum focusing on complex care; behavioral health; teaching on topics in psychology; working with other faculty to coordinate and integrate community health and patient advocacy; understanding resident and faculty support needs; assisting with development of resident learning plans; assisting with wellness efforts; participating in resident feedback and evaluations (including via video reviews); teaching practical office counseling and communication skills to residents; actively participating in faculty and curricular meetings; educating the residency team on available behavioral health community resources; providing real-time care coordination for residents and faculty, and serving as role model for basic counseling skills relevant to the family physician.

+ As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual patients, families and at risk populations.

+ Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

+ The functions of the Social Worker includes crisis intervention, patient /family intervention, high risk screening, counseling, referring for financial or otheridentified resource needs, arrange and facilitate family/patient representative meetings with the health care team as needed, assist in post acute placement on complex discharges and engagement of appropriate agencies or community resources when high risk patients are identified. The Social Worker establishes and maintains professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients and families.

+ The position will be required to serve on hospital committees and take part in developing and presenting in-services to Hospital staff.

Full-time employees can receive up to $18,000 over five years, while part-time employees can receive up to $9,000.

**Qualifications**

**MINIMUM**

Master's Degree from a School of Social work accredited by the Council of Social Work Education.

At least one year of clinical and/or medical social work experience required.

LCSW:AZ

Excellent customer service and presentation skills are a must

Strong interpersonal and written communication skills are essential.

Demonstrated ability to apply analytical and problem solving skills.

Demonstrated ability to manage multiple tasks or projects effectively.

Ability to work independently as needed with a high degree of detail orientation.

Ability to work efficiently in a fast-paced environment with changing priorities.

**PREFERRED**

Counseling or psychotherapy skills preferred.

Prior graduate medical education experience of at least 2 years strongly preferred.

Resourceful, creative, dynamic, and self-directed professional preferred.

**Pay Range**

$33.60 - $48.73 /hour
