Responsibilities Job Summary Under the direction of the Clinical Director or designee, assists in the administrative duties of Case Management to include, but not limited to, the coordination of activities to support the regulatory and documentation necessary for home health patients. The position is focused on supporting the role of the Home Health Case Manager in optimizing the care management process and Case Manager efficiency and effectiveness. Performs a variety of administrative activities. Assists case managers in assigned area of responsibility, including compiling information, organizing paperwork, providing information, coordinating with provider offices, monitoring the status of signed and completed orders. Research, compile, review and maintain a variety of information and records. Answer questions and supply detailed information both in person and on the phone to providers. Deliver or pick up signed orders from providers offices. Respond to provider, patient and nurse inquiries. Fax lab, medication profiles, and discharge and transfer summaries to the appropriate providers. Maintain documents in faxing system and places them accurately into the medical record. Maintains regular communication with the Case Management team & Clinical Director of the status of aging orders. Confers with the Case Management team when a potential conflict arises with a provider. Potential to drive to physician offices to complete administrative functions. Facilitate prompt processing of all requests and tracking of patient supplies for payment. Independently maintains office supply stock and reordering supplies. Performs other duties as assigned. Qualifications Education High School Diploma or GED Required Experience 1 year Clerical support in a healthcare environment Required Licenses and Certifications Driver License Required Proof of Vehicle Insurance Required
